Joining, Placement, Required documents & Affidavit for July 2026 Session

The candidates selected in CIP July 2026 Session are directed to submit the two copies of Joining and Placement Order (In Original) duly signed by the Head of Unit & Assigned Supervisor in CIP Section of PGMIQ Directorate within Seven (07) Days with following documents:

1. Copies of CNIC = 2 Nos
2.  Passport size Pictures : 2 Nos
3.  1 copy of Local Certificate/ Domicile
4. 1 copy of MBBS Degree
5. 1 copy of House Job Experience Certificate
6. 1 copy of CIP Session July 2026  Placement List
7.  Original  Joining Report : 2 Copies
8. Original  Placement Order : 2 Copies
9. 1 copy Letter of Congratulations for (FCPS PGs)
10.  1 copy of Valid PMDC Certificate
11. CPSP Step 1 Foam /Supervisor Consent
12. Affidavit
13. Last Place of posting notification/order/ (if Government employee)
14. NOTE. ALL DOCUMENT MUST BE ATTESTED

Below is the sample of Affidavit required to be attached with the joining and placement documents

AFFIDAVIT

1. It is submitted that I Dr.                                       S/O                                       having CNIC No                               and PMDC No                                 Postgraduate Resident trainee in                    at PGMI/BMCH Session                           .
2. I do hereby solemnly declare that I will not leave the training program unless completion of the program of training as per policy of Government of Balochistan under section 8.2 of PPM.
3. I further declare that i am not enrolled previously any program.
4. If this institute found me previously enrolled in any programme of any institute, my admission will be cancelled without any further notice & will be disqualified.
5. I hereby solemnly I affirms and declare that I will not receive any stipend , Adhoc/Contract salary from the Government of Balochistan, other than the stipend awarded to me by PGMI Quetta for the aforementioned training programme, and I will not draw any financial  compensation until the completion of the training programme.
6. Furthermore; I acknowledge that if I am found to be drawing any stipend , Adhoc/Contract salary from the Government of Balochistan during my enrollment, I will be fully responsible returning the entire stipend received from PGMI Quetta, in such a case, under section 8.2 of the Government of Balochistan policy.
7. That I fully understand and agree to comply with the PGMI Quetta Digital attendance requirements as prescribed by the concerned authority.
8. That I undertake that in case I fail to mark my Ai attendance regularly, I shall not be entitled to receive any stipend for the period of absence.
9. That I accept this condition voluntarily and without any pressure or coercion.

1. The above statement is in best of my knowledge and nothing has been concealed.
